Form NA8 is also called the information form for a temporary residence card for foreigners in Vietnam.

How to fill out form NA8
From number 1 to 10, you can use your personal information combine with some information on your passport to fill out these sections.
1. Full name details (in CAPITAL LETTERS)
2. Gender
3. Date of birth (day/month/year)
4 and 5: Nationality at birth (if any) and current nationaliy.
6. Occupation/position
7. Passport number; type (Ordinary/official/Diplomatic); issuing autority; and expiry date (day/month/year)
8. Your residential and business address befre coming to Vietnam.
9. Date of the latest entry into Viet Nam; via entry port
– Purpose of entry (working, business, traveling, visiting relative….)
10. Pemitted to remain until (day/month/year)
– Current temporary residential address in Viet Nam
– Contact telephone number/Email
11. To issue a Temporary Resident card valid until (day/month/year)
– Reason(s): working as contract, investing or dependent purpose…
The applicant has to sign and write down full name below “người đề nghị”
Some notes to complete form NA8
- Prepare 02 passport photo requires at (2) taken not longer than 6 months, size 2x3cm, white background, straight face, bare head, no glasses.
- (3) and (4) needs to specify the purpose of entry and reason(s) for example: working, studying, investing…
