1. How to fill out form NA5
Form NA5 is divided into 5 main sections:
I. The applicant.
This section has 9 sections, you should open your passport for verification and fill in the required information correctly.
1. Full name details (in CAPITAL LETTER).
2. Gender (Male or Female)
3. Date of birth (day/month/year)
4. Nationality at birth
5. Current nationality
6. Occupation/position
7. Passport or International Travel Document number and passport type (regular passport; official passport; diplomatic passport…)
8. Date of the last entry into Viet Nam (day/month/year) via border checkpoint
– Purpose of entry: traveling, working, studying, visiting relative…
9. Permitted to stay until (day/month/year)
– Temporary residential address in Vietnam
– Contact telephone number/Email
II. Hosting organization/ family relative in Viet Nam.
There are 02 sections in here.
If your visa is categorized as DN1, DN2, LĐ1, LĐ2, ĐT1, ĐT2, ĐT3, ĐT4, DL, TT, LV1, LV2, NN1, NN2, DH, PV, LS… which was sponsored by a hosting organization, you have to complete only part No 1 by filling the name of the organization and their address and contact.
If your visa is categorized TT, 5 years exemption certificate…which was sponsored by a family relative in Vietnam, you have to complete part No 2 and leave part No 1 in the blank.
III. Requests.
There are also 2 sections: visa renewal or visa extension. Depending on your need, you can select one of them then fill in the reasons below.
You should fill only No.2 “to extend the duration of stay until (day/month/year)” when your plan is to stay in Vietnam without leaving the country, during the new visa validity, and leave No.1 blank.
However, if you need a multiple-entry visa, or switch to other types of visa, you have to fill No.1 “to issue a visa”.
Some notes on how to fill in the visa validity:
- Visa DL, DN1, DN2, LV1, LV2, EV…has validity from 1 to 3 months.
- Visa DT 1, DT 2, DT 3, DT 4, DH has a maximum validity of 1 year.
- Visa VR, TT, 5-year visa exemption has a maximum validity of 6 months.
- Visa LD is valid the same as the duration of the work permit.

IV. Additional statements.
This section can be left blank. Finally, the applicant needs to sign and write down their full name below.
If your visa is sponsored by a hosting organization in Viet Nam, their authority has to certify by signature, full name, and stamp.
If your visa is sponsored by a family relative in Viet Nam, the sponsor has to certify by signature, full name.
Some notes when filling out the NA5 form:
- The number of form NA5 need to be submitted is 1 copy enclosed with the original of passport or international travel documents.
- 01 recent taken photo size 4x6cm has a white background, face looking straight ahead, bare head, no glasses.
