How to fill out form NA6
There are two main parts in NA6 form:
Section (1) Full name of hosting organisation
- “Trụ sở tại”: address of the organisation.
- “Điện thoại”: telephone number
Section (2) “Kính gửi” The Immigration Office where the hosting organisation submits your application.
- “Họ tên”: your full name details.
- “Giới tính” Gender
- “Ngày tháng năm sinh”: date of birth (day/month/year)
- “Quốc tịch”: current nationality.
- “Hộ chiếu số”: passport number
- “Vị trí công việc”: position
At the end, “Thủ trưởng cơ quan/tổ chức”: certified by signature, full name, title and stamp.
The validity of each type of temporary resident card (TRC).
- TRC symbols DT1 valid for not more than 10 years.
- TRCs symbol NG3, LV1, LV2, LS, DT2 and DH valid for no more than 5 years.
- TRCs symbol NN1, NN2, DT3, TT valid for no more than 03 years.
- TRCs symbol LD1, LD2 and PV1 are valid for no more than 02 years.
